Team Staff USA Hockey Fee Reimbursement Form

Head coaches, and up to 3 additional rostered assistant coaches, one of which must be a trained goalie coach, will be eligible to receive reimbursement for USA hockey registration, background checks, and level certification requirements.

In order to be eligible for reimbursement, all coaching certificates will need to have been submitted to and accepted by the SPCHA’s registrar within 30 days of teams being announced with the CEP deadline for reimbursement being December 1st.

❗This form is solely for the reimbursement of your personal USA Hockey certification fees required to serve as Team Staff (e.g., Coach, Manager, Trainer).

For all other team or association expenses, please use the General Expense Reimbursement Form.
