USA Hockey Fee Reimbursement Form - Managers and Locker Room Monitors

The SPCHA will reimburse one manager per team for the required NCIS Background Screen, required every other season. Managers should submit all receipts via the reimbursement form below. Each manager must have a current USA Hockey Member number, SafeSport certification, and NCIS background check on file with the SPCHA registrar before they can be involved with players in any capacity. In order to be eligible for reimbursement, managers are required to have these items submitted to and accepted by the SPCHA’s registrar within 30 days of teams being announced.
